New Patient Registration

The area served by the Surgery covers Spennymoor area including the surrounding villages of Kirk Merrington, Hett, Byers Green, Newfield, Binchester and Croxdale.  

We will register new patients resident within the area defined above.   We also take patients who live to the west of the A167 in Ferryhill (Dean Bank area).  (Click to view a map)

You can join the practice by:

• Registering online using this link (opens in a new window)
• Print this form and complete.  Then bring or send to the completed form to Reception
• Call in at Reception and pick up a paper form

Once you have submitted this form, you may need to provide the practice with suitable proof of ID (Passport, Driving Licence, Bank Statement or Utility bill). 

NOTE:  If you have repeat medication at your current practice, please bring a copy of your repeat prescription.

You will be registered with a named GP, but you have the freedom to choose which GP you would like to see.  Simply make known your preference at the time of making an appointment.

Online Registration

Online service makes registration easier for patients

We are using a new online service that makes it easy to register with this GP surgery.

New patients just need to fill in an online form to get started. You do not need proof of address or immigration status, ID or an NHS number. 

Once you have submitted this form. you may need to provide the Practice with suitable proof of ID (Passport, Driving Licence, Bank Statement or Utility bill).

Named GP

As part of the commitment to more personalised care for patients, Practices are required to allocate a named GP to all patients. The named GP will have overall responsibility for their your care and support, with lead responsibility for the co-ordination of all appropriate services.

Where a patient expresses a preference to which GP they have been assigned, the practice will make reasonable efforts to accommodate this request.

Having a named GP does not prevent you from seeing any other Doctor in the Practice. Your named GP will not be available at all times and there may be times when you need to consult with other medical professionals in the practice, which you are free to do at any time.
